Privacy Statement – Therapeutic Centre Pentagon

Therapeutic Centre Pentagon attaches great importance to the protection of your personal data and your privacy. In this privacy statement, we explain transparently what data we collect, why we do so, how we secure it, and what rights you have.
1. Who are we?
Your personal and health data is processed by your psychologist, psychiatrist, psychotherapists, or sexologist within the Therapeutic Centre Pentagon group practice. The psychologists are Charlotte Viaene, Malika Ameziane, Katlijn Vande Perre, Lisa Stockman, Bart Verdikt, Katrien De Decker, Alexander Willems, Stefanie Barigand, Annelies Dierckx, Anastasia Ariefieva, Nelle Dolfen, Thaïs Distelmans, and Funda Caliskan. The psychiatrist is Veerle Jans. The psychotherapists are Lysa Murekatete, Kim Snoeks, Eric Brenael, Anja D’Hauwers, Elma Gaasbeek, and Els Verreck. The sexologist is Erwin Abbeloos, working at Madouplein 8/3; 1210 Brussels and/or Grasmarkt 105/17; 1000 Brussels.
The registration form (via the online contact form on this website) is sent via a secure mail server (www.combell.com) and is only consulted by the coordinator and the psychologist, psychiatrist, psychotherapist, or sexologist who will be providing your counselling.
2. What data do we collect and why?
When you fill in the contact form, we collect the following data in order to process your request:
- Your name, date of birth, address, telephone number, email address, INSZ or national registration number, gender, email address, and telephone number. This is so that we can address and identify you correctly and prevent any duplicate registrations.
- Referrer: For our administration and any communication with your referrer (only with your explicit consent).
- Reason for registration (general): To make an initial assessment of your request for help and to place you with the most suitable therapist.
- Preferred location and therapist: To take your wishes into account as much as possible
Basis for processing: Your explicit consent by completing and submitting the form.
From the first consultation onwards, additional, special personal data (in particular health data) will be processed. This is done in the context of diagnosis and treatment. This processing is necessary for the performance of the agreement (the treatment) and because of an important public interest (provision of healthcare).
- Request for help, personal history, your expectations (your health goals and your living wills), a chronological overview of enhanced healthcare & medications with details of type and date, any complications and the evolution of the condition;
- Details of other relevant practitioners (your general practitioner, previous psychologist, psychiatrist, psychodiagnostics centre, etc.) and details of your confidant;
- Dates of the sessions, including appointments that have been rescheduled or which you did not attend;
- Notes on the content of the sessions;
- Possible information about family members and family history, insofar as this is relevant to the counselling;
- Information that your psychologist receives from third parties that is relevant to the treatment (certificates, referral letters, reports from other treatments, diagnostic reports, test results, advice, etc.);
- Information that your psychologist compiles for third parties (attendance certificates, reports, referrals, etc.);
- Information that you provide to your psychologist in the context of the counselling (homework assignments, personal notes, photos, etc.);
- Information exchanged between you and your psychologist in the context of the counselling (emails, text messages, etc.);
- Possibly photos of assignments, notes on a whiteboard, or personal notes taken during the session.
3. Do we share your data with others?
Your data will be treated as strictly confidential and will not be sold to third parties. We will only share your data in the following strictly necessary cases:
- With the therapist(s) within our practice who are involved in your treatment.
- With your referrer (e.g., general practitioner): only if you have given your prior written consent.
- With the psychologists, therapists, psychiatrist, and sexologist at Therapeutic Centre Pentagon during a consultation. This is done anonymously and with your consent.
- In the event of a legal obligation: for example, in the context of mandatory reporting in cases of acute danger.
- With our IT supplier(s): who have signed processing agreements and do not own your data.
We never share more data than is strictly necessary for the specific purpose.
4. How long do we keep your data?
Data from the application form: If no therapy follows, this data will be deleted after a reasonable period of time (e.g., 3 months).
File data (from the first consultation): In accordance with the code of ethics and legislation, we retain your patient file for 30 years after the last therapy contact, or until at least 10 years after a minor client reaches the age of majority.
5. How do we protect your data?
We take appropriate technical and organizational measures to prevent misuse, loss, unauthorized access, unwanted disclosure, and alteration of your data. This includes, among other things:
- Encryption (SSL) of data sent via the website.
- Secure storage of files (both digital and physical).
- Restricted access to personal data for our employees.
6. What are your rights?
You always have the following rights
- Right of access: You have the right to know what data we process about you.
- Right to rectification: You can have incorrect data corrected.
- Right to be forgotten (erasure): In some cases, you can request that your data be erased.
- Right to restriction of processing: You can request that processing be temporarily suspended.
- Right to data portability: You can request your data in a usable format.
- Right to object: You can object to the processing.
In the event of an exceptional data breach, in which important personal data is leaked, your psychologist will inform you and report this to the competent authorities.
